    I'm trying to purchase the relevant plugin that would help me with a unique single product checkout page (Image Attached). Could you be kind enough to help me choose which plugin I should use?

    Unfortunately, we don't have any plugin that supports checkout form right on single product pages like that. 

    You can consider Instant Checkout feature from our WPC Fly Cart plugin for customers to check out faster on your site.
